VOOR HET MAKEN VAN EEN AFSPRAAK BIJ OPTIMUM MOET U IN NEDERLAND WONEN.

1. Er wordt een aanbetaling van €35 gevraagd om uw afspraak te reserveren. De resterende €60 wordt ter plaatse betaald op de dag van de afspraak. U kunt alleen per PIN of creditcard betalen, geen contant geld geaccepteerd.
2. U ontvangt een bevestigingsmail met daarin een link om uw intakeformulier in te vullen. Deze moet worden ingevuld ten minste 24uur voordat u naar uw afspraak komt. Als deze niet vooraf is ingevuld, kunnen we uw eerste afspraak niet uitvoeren.
3. Wij verzoeken u om een paar minuten voor tijd aanwezig te zijn. Bij te laat komen kan uw afspraak geannuleerd worden.

Uw eerste consult bij ons omvat:

- Intake
- Chiropractisch onderzoek
- Neurologisch onderzoek
- Digitale houdingsevaluatie
- Chiropractische correctie

Het eerste consult duurt 30 minuten.
Bij de 2e afspraak bespreken we uw resultaten, behandelplan en ontvangt u uw tweede chiropractische correctie.
Indien gewenst kunt u eenvoudig direct uw 2e afspraak boeken via een link die u in de bevestigingsmail vindt.

YOU MUST BE A RESIDENT OF THE NETHERLANDS TO MAKE AN APPOINTMENT AT OPTIMUM.

1. A deposit of €35 is required to reserve your appointment. The remaining €60 is paid on site on the day of the appointment. Debit (PIN) or credit card only, no cash accepted.
2. You will receive a confirmation email with a link to complete your intake form. This should be completed at least 24h before your appointment. If it is not pre-filled, we will not be able to conduct your first appointment.
3. We request that you arrive a few minutes early. If you arrive late, your appointment may be cancelled.

Your first consultation takes 30 minutes, it includes:

- Interview
- Chiropractic examination
- Neurological examination
- Digital posture assessment
- Adjustment

At the 2nd appointment, we discuss your results, your treatment plan and you receive your second adjustment.
If desired, you can easily book your 2nd appointment directly via a link that you will find in the confirmation email.

1. Un acompte de 35 € est demandé pour réserver votre rendez-vous. Les 60 € restants sont à régler sur place le jour du rendez-vous. Carte de débit (PIN) ou de crédit seulement, aucun argent comptant accepté.
2. Vous recevrez un e-mail de confirmation avec un lien pour remplir votre questionnaire. Celui-ci doit être complété 24h avant votre rendez-vous. S'il n'est pas pré-rempli, votre consultation ne pourra avoir lieu.
3. Nous vous demandons d'arriver quelques minutes à l'avance. En cas de retard, votre rendez-vous peut être annulé.

Au 2e rendez-vous, nous expliquons vos résultats d'examen, votre plan de traitement et vous recevez votre deuxième ajustement.
